Who You Are
You are a dedicated and registered Social Worker with Social Work England, carrying post-qualification training or significant experience in social work, particularly concerning adults. You possess expertise in safeguarding, mental capacity, and deprivation of liberty legislation, as well as understanding current issues in social work practice, social sciences, and theory.
What the Job Involves
As a Social Worker in the Transfer of Care Team, you will work with Southwark’s NHS partners to support timely and safe discharges for adult service users. In this role, you will engage in comprehensive social work practice, participate in multidisciplinary settings, and conduct complex assessments—including Mental Capacity Assessments and safeguarding inquiries. You will ensure service users are at the center of decision-making, promoting independence and control over their support needs while adhering to statutory guidelines. Mentoring students and less experienced practitioners is also part of your role.
