In this role as an Assistant Practitioner - Occupational Therapist, you will help prevent and reduce social care needs while promoting recovery and independence within the community. You will deliver and monitor therapy plans, provide practical assistance, and work from a strength-based position to enhance individual skills and resources. Your responsibilities include operating within multi-agency settings, carrying out assessments, supporting service users and their families, and maintaining accurate records.
