What the Job Involves
Your key responsibilities include conducting assessments of citizens to meet statutory duties, planning and commissioning services, and developing care and support plans. You will manage a specific caseload, maintain accurate electronic case files, and engage in professional development activities. Additionally, you will work with internal and external partners to coordinate resources and supervise or mentor staff when appropriate. Compliance with council policies and participation in office duties and team activities are essential aspects of the role.
