Who You Are
This role requires an experienced Hospital Social Worker who can think on their feet and manage fast-paced work environments. You should have a solid background, ideally with 4+ years post-qualification experience, in social care settings, especially within hospitals. You should possess an excellent understanding of the Care Act, Mental Capacity Act, and safeguarding practices. As a team player in a multi-disciplinary team at Wexham Park Hospital, you will need effective communication and collaborative skills, a commitment to person-centered care, and the ability to support vulnerable individuals through personalized interventions.
What the Job Involves
You will be responsible for ensuring the safe and timely discharge of service users from hospital to the community by working collaboratively with health partners. The role involves holding a complex caseload, conducting holistic assessments, and implementing care and support plans. You will work with various professionals, contribute to joint working practices, and adhere to policy for smooth transitions. Additionally, you will manage preventive measures to reduce readmissions and support ongoing recovery, while also dealing with safeguarding issues and maintaining high data quality standards.
