Role

Who You Are

You are a dedicated Social Worker with experience in community care, particularly with vulnerable adults, in either community or hospital settings. You have a strong understanding of social services legislation and are committed to promoting independence and safeguarding adults at risk. Your interpersonal skills allow you to build relationships and effectively communicate with a variety of people. You are a proactive team player with excellent organizational skills and the ability to manage workload efficiently. You hold a relevant qualification in Social Work and are registered with the Health and Care Professionals Council.

What the Job Involves

In this role, you will promote and maintain the independence of vulnerable adults while safeguarding them from abuse. Your responsibilities include assessing needs, formulating personal budgets and support plans, and monitoring services to ensure they meet individual needs. You will participate in safeguarding inquiries and Best Interest Assessments, provide assessments for individuals and carers in line with the Care Act 2014, and coordinate care plans with professional colleagues. You will also supervise and support staff, offer expert advice, manage budgets, maintain client records, and contribute to service development.
