You are an experienced and dedicated Social Worker with a strong understanding of the needs of adults within the community, particularly those with learning disabilities. You are committed to using strength-based approaches to empower individuals and enhance their well-being. You thrive working within multi-disciplinary teams, building strong relationships with colleagues, external agencies, and service users.
What the Job Involves
This role is part-time (18.5 hours per week) and focuses on conducting annual reviews, managing non-complex care cases, and completing Community DoLS assessments. You will work within the Community Team for People with Learning Disabilities, using a strength-based model of social care in a locality setting. Your responsibilities include supporting self-assessments, facilitating community service identifications, ensuring safe equipment use, and liaising with professionals to meet service user needs. You will record and respond to safeguarding issues and contribute to evidence-based practice.
Skills
- Strong knowledge of the Care Act and Mental Capacity Act
- Experience with community and asset-based conversations
- Ability to conduct risk, need, and capacity assessments
- Competency in financial and support planning
- Excellent case management and organizational skills
- Ability to build strong relationships with multi-disciplinary teams
- Proficient in the use of electronic IAS systems
- Strong safeguarding and duty work capabilities
