Who You Are

You are a compassionate and skilled Social Worker who specializes in transitions from Children’s Services to Adult Services. You have experience in supporting young adults with disabilities, and are comfortable working with individuals aged 16-25. You thrive in a collaborative multi-agency environment and are committed to making a positive impact on young people’s lives as they transition into adulthood. You must have the ability to drive as this role involves covering all of Worcestershire and some national placements.

What the job involves

As a Social Worker within the All Age Disability Service's Young Adults Team, you will support young people with disabilities aged 16-25 in their Preparation for Adulthood. This involves helping them in areas such as education, employment, independent living, community inclusion, and health. You will work primarily with young people with learning disabilities, but also with those who have physical disabilities or complex health needs. Your role will involve collaborating with Worcestershire Children Services, Adult Services, the Special Educational Needs Department (SEND), health colleagues, schools, and colleges to ensure comprehensive support for these young adults. You will use Liquidlogic in both adult and children’s systems, and work within children and adults legislation effectively.
