Who You Are
You are a qualified social worker, registered with Social Work England, with a passion for supporting young people with disabilities in their transition from childhood to adulthood. You possess strong interpersonal skills and a deep understanding of social work theories and legislation related to children and young adults. You have experience in statutory social care settings, safeguarding, and working with individuals with disabilities.
What the Job Involves
As a Social Worker in the All Age Disability Service, you will work with young people aged 16-25 with learning and physical disabilities, aiding their transition from children’s services into adult life. You will develop comprehensive care plans in collaboration with families and multi-agency partners, ensuring the best outcomes in education, independent living, community inclusion, and health. Your role involves safeguarding vulnerable individuals, assessing risks and needs, and coordinating services. You will maintain accurate records and participate in training for professional development.
Skills
- Professional social work qualification with SWE registration
- Specialist knowledge of social work theories and practice
- Detailed knowledge of current children and young adult legislation
- Strong interpersonal and communication skills
- Experience in safeguarding and risk assessment
- Ability to work effectively in a multi-disciplinary team
- IT proficiency, especially in Microsoft Office
- Ability to travel across Worcestershire and beyond, if required
