You excel in applying a 'whole family' approach to prevent the need for statutory services and ensure that families access universal services. You are comfortable working closely with children across various age ranges and have experience working with SEND procedures. Your role requires an enhanced DBS check.
What the job involves
The Outreach Family Support Worker takes responsibility for complex cases stepping down from children’s social care. You will conduct early help assessments, arrange and lead Team Around the Family meetings, and provide intensive, targeted support to vulnerable children and young people. You will act as the Lead Professional when required, developing and reviewing family plans, applying de-escalation techniques, and promoting parenting strategies. Your work aligns with the Signs of Safety model and involves collaboration with other professionals to ensure positive outcomes. Additionally, you are responsible for maintaining accurate records, engaging with service users, promoting equal opportunities, and ensuring safeguarding of children and young people.
