Who You Are
We are looking for a highly skilled and confident social worker to join our dynamic hospital discharge team. You should have more than 5 years of post-qualification experience in adult social care, ideally with expertise in hospital discharge or Discharge to Assess (D2A) processes. You are a self-starter who can progress cases independently without step-by-step guidance. You possess a strong understanding of the Care Act, Continuing Healthcare (CHC), and safeguarding, along with excellent decision-making, risk management, and multi-agency working skills. You are a calm and confident communicator who thrives in fast-paced environments.
What the Job Involves
Leading on complex discharges from hospital settings, coordinating timely and appropriate long-term care solutions. You will be responsible for navigating assessments, funding pathways, and family dynamics with professionalism and empathy while ensuring legal and policy compliance through a person-centred approach. This role requires the ability to manage a caseload of complex cases and provide case management, guidance, support, and expert advice to social care staff and students. Additionally, you will need to travel within the local area regularly for assessments and meetings related to Continuing Healthcare (CHC) and safeguarding (SG) initiatives.
