What the Job Involves
This role involves working as part of the Mental Health Social Work Discharge Team to coordinate and manage discharges for complex social care cases from acute and community mental health hospitals. You will complete statutory work such as Care Act assessments, s117 aftercare planning, safeguarding, mental capacity assessments, and participate in Best Interest Meetings. You will collaborate with health colleagues and use your social work skills to develop support plans that focus on improving wellbeing and achieving outcomes. You will work within a supportive environment that encourages professional growth and provide informal supervision for less experienced practitioners.
