You thrive in a multidisciplinary environment and are ready to take on challenges on Wednesday, Friday, and assigned training or duty days.
What the Job Involves
This position involves performing social care needs assessments and engaging in support planning and commissioning. You will be responsible for risk assessing and managing risks, which includes attending multi-agency risk assessment meetings (MARAMs), ensuring safeguarding, and understanding the Mental Capacity Act (MCA), Best Interest (BI) decisions, Court of Protection (COP) cases, and Deprivation of Liberty Safeguards (DOLS). You will also manage Continuing Healthcare (CHC) and Decision Support Tool (DST) evaluations.
