Job Description: Reablement Occupational Therapist

Who You Are

You are a compassionate and dedicated occupational therapist with a focus on supporting individuals in regaining their independence through community-based interventions. You demonstrate adaptability and resilience, with a readiness to work flexible shifts, including early mornings, late evenings, and weekends. Your proactive approach and excellent organizational skills allow you to effectively manage your time and workload, fostering positive outcomes for those you support.

What the Job Involves

This role involves delivering community-based reablement services as a part of a multidisciplinary team. You will be required to assess, plan, implement, and evaluate care plans to enable clients to achieve their rehabilitation goals. The position requires travel within the community; hence, daily use of a car is necessary. The role follows a rotational shift pattern with 2 to 3 days of early (8am to 4pm) and late (12pm to 8pm) shifts, along with weekend shifts (9am to 5pm) every 6 or 7 weeks. A hybrid working arrangement allows for a mix of community visits and remote work, with an annual rota provided to assist planning.
