What the Job Involves
As a SEND Lead Worker, you will act as the key point of contact for young people with SEND and their families, coordinating activities required to assess needs and ensure holistic care and support. You will develop appropriate care plans, collaborate with multidisciplinary teams, and facilitate the Education, Health, and Care Needs assessment process. Your role includes promoting early identification of SEND needs and ensuring services are arranged efficiently and effectively.
