Who You Are
We are seeking a suitably qualified, innovative, enthusiastic, and highly motivated Senior Social Worker to join our Hayes and Five Elms Locality Team. You are committed, flexible, and hardworking with a passion for maintaining independence for vulnerable adults and their carers. You should excel in a multidisciplinary framework, possess good communication and negotiating skills, and have a strong ability to work with partner agencies.
What the Job Involves
The Senior Social Worker role focuses on ongoing care management interventions, annual reviews of support plans, extra care housing support, and direct payment work. You will conduct assessments under the Care Act 2014 and support transitions from Children's to Adult services. Responsibilities include promoting independence for adults, safeguarding work, assessment and implementation of personal budgets, and supervisory duties for both qualified and unqualified care managers. You will need to support and advise team members and deputize for the Team Leader.
