Who You Are
An experienced social worker with a strong background in assessing social care needs of adults, especially the elderly and people with disabilities. You have excellent knowledge of current social services legislation, particularly relating to adults, and are proficient in safeguarding and related legal frameworks. You are registered with Social Work England, and thrive in multi-agency settings, with a focus on promoting independence, recovery, and holistic assessments.
What the Job Involves
The role is part of the Intermediate Care (Reablement) Service in Southwark, reporting to the Reablement Team Manager. You will conduct Care Act assessments, devise care and support plans, chair Best Interests meetings, conduct crisis interventions, and work with individuals with complex needs. The job involves safeguarding responsibilities, including initial fact-finding and risk management. You will collaborate closely with health professionals in a multi-disciplinary setting, providing advice and information to service users and their families, and facilitating access to community resources.
