You are a dedicated advocate for young people transitioning from public care. You excel in building partnerships with various stakeholders, are passionate about promoting the welfare of young people, and have a strong commitment to meeting the needs associated with diversity, including religious, racial, gender, cultural, and linguistic backgrounds.
What the job involves
The role involves collaborating with young people, their carers, and key agencies to deliver an integrated holistic service in line with relevant legislation. You will develop strong relationships with multi-agency partners to coordinate responses to the needs of care leavers. Responsibilities include coordinating, monitoring, and reviewing Pathway Plans for young people transitioning to independence, supporting them to maintain accommodation, preventing rent arrears, engaging with benefit agencies, and ensuring appropriate entitlement to services. You will actively support inter-agency pathway planning.
