You are a motivated and compassionate individual looking to join the Adult Services Review Team as a Social Work Assistant. You should be educated to GCSE standard or equivalent, with experience working with older people and adults with physical disabilities. You are committed to equality of opportunity and empowering service users.
What the Job Involves
As a Social Work Assistant, you will work under the guidance of the Team Manager to arrange and monitor services according to individual care plans, provide advice and practical help, and liaise with various agencies. You will also be responsible for completing annual care plan reviews, participating in team meetings, and maintaining alignment with Council Policies and Procedures. You will be predominantly office-based at County Hall but may be required to visit service users when needed.
