Who You Are
You are a compassionate and experienced social worker committed to supporting individuals during their transition from hospital care to community support. With a minimum of 2 years of experience in Adult Social Care, you are adept at working collaboratively with residents, their support networks, and other professionals.
What the Job Involves
This role involves conducting care and support assessments for residents who have recently been discharged from the hospital under the Discharge to Assess program. You will collaborate closely with the residents, their support networks, and multidisciplinary teams to create and implement personalized care plans, ensuring a smooth transition to community living. The contract is ongoing, with an initial term of 3 months.
