Case Worker – Occupational Therapist WCR 3
City of Westminster Council
Role
Who You Are
We are looking for a skilled and experienced Occupational Therapist to join our Adult Social Care team at Westminster City Council. You should be committed to delivering high-quality, person-centred interventions for residents with complex needs. Your expertise will support clients to live safely and independently in their homes through functional assessments, equipment provision, and major adaptations. A proactive problem-solver, you will engage with clients, colleagues, and partner agencies to provide coordinated service delivery.
What the Job Involves
This role involves managing a caseload of complex cases, conducting comprehensive home visits, and completing manual handling assessments. You will prescribe and oversee major adaptations, liaising with contractors and coordination with housing and DFG teams. Clinical reasoning skills are essential in justifying high-cost equipment and structural changes. You will contribute to hospital discharge planning and ensure timely provision of essential equipment to facilitate hospital avoidance. Documentation, supervision of junior staff, and acting as a main point of contact for service users are also key components of this role.
