Who You Are

You are a dedicated professional in the field of social care with experience or qualifications at NVQ level 3 or equivalent. You possess a deep understanding of the legislation and current issues affecting the provision of Community Care Services and have specialized knowledge in manual handling techniques and adaptations for people with disabilities. You are committed to continuous professional development and exhibit excellent teamwork, customer focus, and communication skills.

What the Job Involves

As an Assessment Officer, you will report to the Team Manager and work collaboratively with various directorates, health services, and organizations to deliver exceptional social care services. Your role will include conducting assessments, recommending rehabilitation techniques, planning support, and reviewing plans for vulnerable and disabled individuals. You will manage caseloads, liaise with customers and other professionals, and ensure compliance with statutory obligations. Your contributions will be vital in optimizing service delivery and facilitating integrated assessments.
