Who You Are
A dedicated professional with experience in working with adults with learning disabilities, ADHD, and Autism. You have completed your ASYE and have at least two years of post-qualifying experience. You are committed to promoting independence, prevention, and self-directed support for vulnerable adults and their carers. Risk management and safeguarding are key areas of your expertise, and you are adept at integrating service users into community-based solutions.
What the Job Involves
This role involves conducting annual reviews to assess the needs of vulnerable adults and their carers, aiming to support them in arranging services to meet identified needs and outcomes. You will be required to complete up to three reviews per week, all conducted face-to-face. The job focuses on ensuring timely and accurate assessments, promoting service users' independence and recovery, and adhering to risk management and safeguarding procedures. Collaboration with multi-agency teams and community development staff is essential to provide coherent health and social care services.
