You are an experienced and motivated individual with a background in social work, particularly in fast-paced environments such as hospital social work. You are flexible and willing to work across various sites and locations. You have a passion for promoting independence, advocating for service users, and working collaboratively with other professionals to achieve the best outcomes for individuals.

What the Job Involves

You will provide a first response service for individuals accessing social care, offering advice, information, and support. Your role includes conducting assessments, promoting independence through the use of resources such as Reablement and assistive technology, and ensuring that individuals and their families are at the center of decision-making. You will collaborate with social workers, occupational therapists, and other relevant professionals to ensure seamless transitions and effective service delivery. Additionally, you are responsible for maintaining accurate records and managing a caseload in accordance with established policies and procedures.
