Who You Are
The Community Team for People with Learning Disabilities (CTPLD) is looking for a motivated and qualified Social Worker to join our dynamic, multi-disciplinary team. Based at Observatory House, Slough, you should possess the relevant qualifications for a Social Worker and have experience working within an Adult Learning Disability team. Familiarity with the Liquid Logic (LAS) case management system is also desirable.
What the Job Involves
As a Social Worker with CTPLD, you will undertake Care Act (2014) assessments, risk assessments, and support planning to identify and meet eligible needs. Duties include completing Mental Capacity Act (2005) assessments, making best interest decisions, managing complex care and support cases, and contributing to Continuing Healthcare (CHC) processes. You will also prepare and submit Community Deprivation of Liberty (DoL) applications, and collaborate with health and social care professionals to promote person-centred support. The role offers hybrid working arrangements with a minimum of two days in the office including participation in a duty rota. The position is full-time, weekdays from 0900 to 1700hrs, with a pay rate of £37 to £38/hr.
