Who You Are
You are committed to making a positive impact on the lives of young people transitioning from public care. You have extensive experience working with young people, including those at risk, in various settings. You possess qualifications in relevant areas such as Youth Work, Careers, Social Services, or similar fields, and are open to undertaking additional training. You have strong core behaviors including focusing on people, taking ownership, working collaboratively, communicating effectively, and focusing on results.
What the Job Involves
The role requires working in partnership with young people, their carers, social workers, and various agencies to deliver a holistic Personal Advisor and Aftercare Service. You will be responsible for coordinating, monitoring, and reviewing Pathway Plans for care leavers, advocating for their needs, and supporting them in developing skills for independent living. This involves ensuring they have adequate accommodation, access to welfare benefits, and engaging with multi-agency partners for successful transitions to adulthood.
