What the Job Involves
You will be responsible for providing and coordinating advice and support to care leavers. You will participate in assessments, prepare pathway plans, and design interventions for young people leaving care. Your role involves ensuring these plans are implemented effectively and keeping detailed records of your interactions and the services provided. You'll also be expected to challenge discriminatory behaviour and report any concerns related to the welfare or safety of young individuals.
