You are an adaptable and compassionate individual, dedicated to supporting adults and older people through direct interaction. With a commitment to equality and respect for others, you bring experience in working with diverse agencies and possibly within hospital settings. You possess a strong educational background, ideally at least GCSE standard or equivalent, with some vocational qualifications in social care.

What the Job Involves

This role, positioned within the Adult Services Review Team at County Hall, involves arranging and monitoring services aligned with individual care plans. Responsibilities include providing advice, practical help, liaising with voluntary and statutory agencies, and completing annual care plan reviews. You'll actively participate in team meetings, duty rotas, and champion Council policies. While mostly office-based, some visits to service users may be required.
