Who You Are
You are a dedicated and compassionate individual with experience in working with older people and/or individuals with disabilities. You possess a QCF Level 2 in Health & Social Care and are registered with Social Care Wales. You have experience in line-management and maintaining compliance within a regulated service. You are a capable communicator, both in writing and verbally, with a person-centered, strength-based approach.
What the Job Involves
As a Reablement & Bridging Senior Care Officer, you will manage a team of Reablement Care Workers, providing supervision and ensuring quality care delivery. You will support more complex client cases to promote their independence at home after illness or hospital stay. The role involves working collaboratively with the Community Resource team and ensuring all records and training levels are up to standard. The role includes mentoring staff, promoting independence in service users, and adapting to flexible work locations. You will also support the Council’s initiatives on equality, carbon reduction, and safeguarding.
