What the Job Involves
This Social Work Assistant role is based at County Hall within the Adult Services Review Team. You will work closely with team members, including the Team Manager and Social Workers, to arrange and monitor services in line with individual care plans. Responsibilities include providing advice, conducting annual care plan reviews, and liaising with various agencies. You will participate in team meetings and adhere to Council policies, with potential work across different locations as needed.
