Who You Are
You are a compassionate and committed care management social worker, dedicated to assessing and addressing the needs of vulnerable adults and their carers. You prioritize promoting independence, self-directed support, choice, and control among service users. You are also vigilant about risk management and safeguarding, ensuring that all practices are in line with both adult and children's safeguarding procedures where relevant.
What the Job Involves
The role involves conducting timely and proportionate care management assessments to identify the outcomes desired by service users and their carers. You will work as part of multi-agency teams to provide integrated health and social care services, promoting service user independence, recovery, and community integration. Responsibilities include ensuring adherence to risk management and safeguarding procedures, involving service users and carers in creative support planning, and working with community development staff to facilitate access to community-based services. You are expected to manage case work, uphold rigorous documentation standards, and supervise a team member, fostering self-help and strengthened community capacity.
