Who You Are
You are a committed individual with a caring nature, equipped to support citizens in Cardiff to live independently at home. You possess good observation and recording skills, and have a flexible, focused approach to work within a dynamic and fast-changing environment. You have the ability to train for the relevant HSc level required for the role and are ideally able to speak additional languages, such as Welsh or Community Languages.
What the job involves
As a part of The Community Resource Team (CRT) Homecare, you will support clients in Cardiff, preventing unnecessary hospital admissions and facilitating discharges. You will work closely with health professionals, using a service delivery plan to guide your daily tasks, as allocated by a mobile device. The role involves various rotas and hours, including evenings and weekends. You must be committed to achieving the wellbeing goals of service users through reablement support.
