Role

Who You Are

We are looking for dynamic, skilled, and highly motivated Social Workers to join our team. You should have a passion for social care and promoting independence. You will work at Bassetlaw Hospital and Retford Hospital, with an expectation of being on site two days per week.

What the Job Involves

As part of the Urgent & Emergency Care Team (UEC NORTH), you will maintain a complex caseload, be responsible for the assessment, support planning, and review of individuals' needs, and coordinate outcomes to promote independence. You will undertake assessments using various health and social care tools and identify Reablement opportunities. Promoting a Strength-Based Approach, you will ensure people can assess their own needs and meet their goals, delaying long-term care. You will liaise with local providers, offer advice and support to individuals and caregivers, and undertake roles such as Safeguarding Officer, AMHP, BIA, or ACMP when trained. You'll contribute to practice development, mentor new staff, organize your workload independently, and respect confidentiality at all times.
