Who You Are
You are dedicated to supporting children, young people, and their families by providing high-quality, outreach, and home-based Early Help support. You employ a "whole family" approach, working with children across various age ranges to prevent the need for statutory services. You have an understanding of the SEND Code of Practice for young adults (ages 19-25) and are committed to promoting equal opportunities and safeguarding children and young people.
What the Job Involves
The Outreach Family Support Worker acts as a case holder for complex cases that have stepped down from children’s social care, providing targeted support and coordinating Team Around the Family plans. You will support families to improve outcomes for their children, offering interventions such as bespoke programs, risk assessments, and de-escalation techniques, while aligning work to the Signs of Safety model. You'll also maintain effective records on electronic systems, attend supervision sessions, and contribute to service evaluations and new developments. This role requires home visits and active participation in meetings, with an emphasis on continuous professional development and adherence to safeguarding legislation.
