What the Job Involves
This role involves coordinating patient discharges from an acute care hospital setting. As an on-site social worker, you will assess the psychosocial needs of patients, create comprehensive discharge plans, and link patients and families with community resources. You will work closely with doctors, nurses, and other healthcare professionals to ensure integrated care delivery and optimal transition plans for patients. The position requires proactive problem-solving to address potential barriers to discharge and ongoing patient support post-discharge.
