You are responsive, efficient, and capable of working independently to manage referrals and coordinate services effectively.

What the Job Involves

Your main responsibility is to coordinate the brokering of services for residents with social care needs. This involves ensuring that the care commissioned is appropriate and offers value for money. You will facilitate hospital discharge referrals in accordance with the NHSE guidance, ensuring responses are made swiftly, especially within the 3-hour window post-optimization for discharge. Additionally, you will manage community referrals promptly to source and secure suitable care settings. You are required to work Tuesday to Friday from 9am to 5.15pm, Saturday from 9am to 4pm, and on Bank Holidays from 9am to 4pm.
