What the Job Involves
This is an exciting opportunity to be based within a local authority integrated health and social care service. The service comprises of many disciplines including Social Work, Community Nurses, Psychologists, Occupational Therapists, Speech and Language Therapists, Art Therapists, Physiotherapists, and Psychiatrists. The Care Coordinator post will sit within the social work team.
The role entails face-to-face work with service users, their families, and external partners. The purpose of the Care Coordinator role is to identify whether support packages continue to meet the needs of service users and their families. The role will require the successful candidates to make recommendations regarding support packages and arrange and amend support to ensure needs are being met in a person-centred and strength-based way.
