What the Job Involves
Your role involves conducting strength-based assessments and support plans, undertaking safeguarding inquiries, and managing a complex caseload. You actively engage with carers and work collaboratively with health-related agencies and other partners to ensure high-quality outcomes. You promote independence by utilizing reablement and assistive technology, and you conduct ongoing assessments of need and risk, ensuring compliance with statutory requirements and good practice standards.

- Working knowledge of national policies and legislation (Care Act 2014, Mental Health Acts)
