Social Worker (Grade 11)

Who You Are

We are seeking an experienced agency social worker to join the Rhondda Cynon Taf West Learning Disability Team. You will be part of an experienced and supportive team working primarily in the Rhondda area, dedicated to supporting adults with learning disabilities.

What the job involves

The role involves safeguarding and promoting the wellbeing of adults through high-quality social work in assessment, care planning, and review. You will conduct assessments per the Social Services and Well-Being Act (Wales) 2014, develop care and support plans, and monitor and review these plans. You will perform Mental Capacity Assessments in line with the Mental Capacity Act 2005, assess risks to promote independence and choice, and work collaboratively with other agencies using a multi-disciplinary approach. Accurate electronic record-keeping in line with departmental policies is essential. You must be able to travel independently within the service area and should reside within a reasonable distance to fulfill these duties. The team operates remotely with one office day per week.
