Social Worker (Grade 11)

Who You Are

We are seeking an experienced agency social worker to join our team focusing on adults with learning disabilities in the Rhondda Cynon Taf East area. You should be familiar with serving the region from Pontypridd to Aberdare/Hirwaun, and comfortable collaborating within a supportive and experienced team.

What the job involves

The role involves safeguarding and promoting the well-being of adults through high-quality social work practices. Responsibilities include conducting assessments per the Social Services and Well-Being Act (Wales) 2014, identifying outcomes, and developing, monitoring, and reviewing care and support plans. You will perform Mental Capacity Assessments and work within the guidelines of the Mental Capacity Act 2005. Risk assessment, promoting individual strengths, collaborating with other agencies, and maintaining accurate electronic records are key duties. You will need to travel independently throughout the service area with primarily remote work and one office day per week.
