Who You Are

A committed and compassionate professional responsible for supporting the delivery of health and social care functions for adults and carers in Wirral. This role is crucial in promoting Personalisation and Inclusion Pathways for Adults and Carers, while ensuring the effective application of the Care Act 2014 and related legal frameworks. The ideal candidate should have a social work qualification, be registered with Social Work England, and demonstrate continuous professional development.

What the Job Involves

This position requires you to assess, access, and implement a range of social care services. You will provide guidance, advice, and information to adults with complex long-term physical or mental health needs, older people, and their carers. Duties include working as part of an integrated team, carrying out assessments, planning and reviewing care, safeguarding vulnerable adults, and participating in community forums and multi-disciplinary meetings. You will also need to challenge societal stigma, submit funding applications, manage performance, comply with corporate policies, and uphold professional values.

Skills

  • Knowledge of relevant legislation: Care Act 2014, Mental Capacity Act 2005, etc.
  • Ability to communicate and engage in complex/challenging situations
  • Strong organizational skills and ability to manage competing demands
  • Proficiency in IT, e.g., Microsoft Office
  • Experience with safeguarding and multi-disciplinary work
  • Ability to work with vulnerable adults and carers
  • Knowledge of confidentiality and data protection requirements
  • (Desirable) Ability to prepare statements for court proceedings
  • (Desirable) Best Interest Assessor training
