What the job involves
You will undertake complex assessments, develop and review care plans, and manage cases of high complexity. Your role involves ensuring legal and statutory compliance, promoting independence, and preventing unnecessary hospital admission. You will work in an integrated team, managing resource allocation, providing professional guidance, and contributing to service improvements. You will be responsible for risk assessments, lead communications, and represent your team in multi-disciplinary settings. Mentorship, staff development, and active participation in recruitment and learning initiatives are also integral to the position.
