What the Job Involves
This role involves conducting comprehensive assessments and designing care and support plans tailored to client needs. You will manage an allocated caseload, escalate complex cases, and provide professional support and advice. You will work closely with individuals, families, and carers to assess risks and resources, collaborate on care plans, and ensure the effective use of available services. Participation in a duty system rota, involving immediate crisis response and effective case management, is also required.
