You are an experienced professional passionate about working within the Community Mental Health Team at East London NHS Trust. You are committed to providing exceptional social work services and making a positive difference in the lives of service users and carers.
What the Job Involves
This role requires delivering a comprehensive social work service under the Care Programme Approach and the Care Act, as well as other relevant legislative frameworks. You will work with individual service users both in hospital settings and within the community. Your work will include providing advice, assistance, assessments, reviews, and other forms of social work service, all in line with Social Work England guidelines, with the ultimate goal of enhancing outcomes for service users and their carers.

- Strong understanding of the Care Programme Approach & Care Act
- Proven experience in social work services
- Ability to work collaboratively in a healthcare team
