You are a dedicated and empathetic professional holding a recognized Social Work qualification, registered with Social Work England. You have relevant multidisciplinary Social Care experience within a team environment and demonstrate an understanding of Social Care practices. You possess IT skills necessary for effective data management and are fluent in English. You have personal qualities, such as sensitivity, reliability, and resilience, along with the capability to work under pressure and adapt to change.
What the Job Involves
This role involves assessing risk and care needs of service users and carers, managing a caseload with varying complexity, and engaging with individuals who refuse service but are at risk. You will conduct statutory reviews, prepare reports, and maintain high-standard client records. The role includes collaborating with other professionals across various agencies to provide needs-led services and support independence in service users. You will also supervise and mentor less experienced staff and participate in continuous professional development.
