Who You Are
As a compassionate Social Worker in Adult Services, you bring relevant experience working with adults with learning disabilities and autism. You are knowledgeable about the social model of disability and demonstrate empathy, sensitivity, and self-awareness. You are self-reliant and resilient, capable of prioritizing workload and managing competing deadlines. You embrace organizational change and maintain professional relationships with service users and other professionals to deliver effective partnerships. Access to a vehicle is essential, and you possess the ability to speak fluent English as required by the Immigration Act (2016).
What the Job Involves
This role involves assessing the risk and care needs of service users and carers and developing care plans that reflect individual circumstances. You will manage a caseload, respond to referrals, and engage with people at risk. You'll coordinate with other professionals, understand the needs of carers, and negotiate the purchase of care provision when necessary. Statutory responsibilities include managing crisis situations, preparing reports, and collaborating with primary care, police, and other agencies. Additionally, you'll participate in team meetings, support your own professional development, and contribute to policy and practice improvements.
