Who You Are

As a dedicated and caring individual, you possess exceptional observation and recording skills. You have a flexible, focused approach and can adapt to work within a dynamic, fast-changing service. You're committed to supporting service users to achieve their reablement goals, enhancing their independence with a caring nature. Holding or willing to train to obtain the relevant HSc level, your ability to communicate in multiple languages, particularly Welsh and Community Languages, is advantageous. You must have a full clean driving license and access to a car.

What the Job Involves

You will play an essential role in supporting the citizens of Cardiff, working as part of The Community Resource Team (CRT) Homecare to prevent unnecessary hospital admissions and facilitate hospital discharges. Your goal will be to assist people to live independently in their own homes by working closely with health professionals and following a personalized service delivery plan. You will conduct assessments with service users, develop plans to meet their individual needs, and work shifts as per an allocated rota while managing tasks through a mobile device. The position offers various working hours, including evenings and weekends.
