Who You Are

You are a dedicated and caring individual with a commitment to supporting the citizens of Cardiff. You possess good observation and recording skills, along with a flexible, focused approach necessary to adapt within a dynamic work environment. You have or are willing to achieve the relevant HSc level. Multilingual capabilities, particularly in Welsh and Community Languages, are advantageous. You must have a full clean driving licence and access to a car.

What the Job Involves

As part of the Community Resource Team (CRT) Homecare, you will help prevent unnecessary hospital admissions, facilitate discharges, and assist individuals in living independently at home. This involves working closely with health colleagues to support the service user’s reablement goals. You will follow a service delivery plan to support clients in achieving their wellbeing goals, working in shifts based on a rota. Communicating updates and progress requires using a mobile device for task allocation. Various rotas, including evening and weekend work, are available.
