Role
Who You Are
You are an experienced professional with a background in adult social care, community occupational therapy (OT), hospital discharge, reablement, or similar settings. You thrive in fast-paced environments, excel at managing complex cases and risks, and are comfortable completing assessments remotely. Your skills are rooted in a strong understanding of the Care Act, housing legislation, equipment provision, and adaptation processes. Communication, organization, and problem-solving are your strengths, and you are committed to promoting equality, diversity, and inclusive practice.
What the Job Involves
