What the Job Involves
This role involves managing a responsive brokerage service to implement Support Plans in a person-centered way, ensuring the best value for money. You will provide an urgent duty brokerage service to meet urgent needs, including hospital discharges, with a flexible work schedule, which may include evenings, weekends, and public holidays. Additionally, you will serve as the crucial link between care managers and care providers, working with care management and commissioning teams. You will also be responsible for offering ongoing capacity building support to your team.
