Role

Who You Are

You are a compassionate and dedicated individual committed to supporting disabled young people with complex needs as they transition to independent living. You understand the importance of social inclusion and empowerment and are eager to collaborate with social workers and other colleagues for the benefit of the youth. You are adaptable, able to manage scarce resources effectively, and willing to work flexibly, including evenings and weekends, as required.

What the job involves

This role involves managing and providing personalized advisory support to disabled young people aged 16+ who are preparing to leave care and transition to independence. You will ensure they have suitable accommodation, access to education or training, and are not socially isolated. Responsibilities include acting as an advocate, quality assurance of pathway plans, financial assessments, and coordination with educational and community partners. You will maintain statutory records while safeguarding confidentiality and data protection.

Skills

  • Knowledge of relevant legislation (Children (Leaving Care) Act 2000, Social Services and Well-being (Wales) Act 2014)
  • Experience with multi-agency collaboration
  • Strong interpersonal and empathetic communication
  • Ability to assess and manage social care needs
  • Understanding of child protection and adolescence development
  • Proficient in IT systems and record keeping
  • Ability to travel independently within Rhondda Cynon Taf
  • Welsh Language Level 1 or higher
